The purpose of this RFP is to solicit offers for a data management system that fulfills federal regulations for assessing Residents, care planning, and documenting care delivery for the North Carolina Department of Health and Human Services (NCDHHS) Division of State Operated Healthcare Facilities (DSOHF) (hereinafter referred to as “Agency”) and its three (3) Neuro Medical Treatment Center (NMTC) facilities. The NMTCs are specialized skilled nursing facilities certified by the Centers for Medicare and Medicaid Services (CMS) under the Omnibus Budget Reconciliation Act long-term care regulations. The Care Plan Management (CPM) system (hereinafter referred to as the “Solution”) will support clinicians with the performance of required assessments and care plan development to identify each Resident’s functional and health care needs and to track care delivered in accordance with the plan. Currently the NMTCs use a hybrid solution with paper processes and various software solutions that are not integrated, resulting in staff spending countless hours performing redundant data entry and administrative tasks versus delivering Resident care.

Either State-hosted or Vendor-hosted options for the platform and data can be proposed by the Vendor based on the optimal solution the Vendor plans to offer. Vendors may propose alternative development approaches according to their business model and technical delivery approach. Because either a commercial off-the-shelf (COTS) with State or Vendor hosting or a Vendor-hosted software-as-a-service (SaaS) solution is acceptable, the terms applicable to a State-hosted Solution and a Vendor-hosted Solution are both included in this RFP, in Attachment B: Department of Information Technology Terms and Conditions. Section 2  of the DIT Terms and Conditions applies to a “COTS” solution while Section 4 applies to a SaaS Solution. The agency will retract the inapposite licensing terms during any negotiation phase of this procurement.
